What is Family-Managed Home Care?
The Family-Managed Home Care (FMHC) Program, also known as Self-Directed Care, offers families greater control and flexibility in managing home care services. Eligible patients or their substitute decision-makers (SDMs) receive direct funding from Ontario Health atHome to:
- Hire caregiversfor personal support, nursing, and other essential services.
- Manage home care providersand funding to meet the needs outlined in the patient’s care plan.
This program is ideal for families who want to tailor care to their unique circumstances while maintaining compliance with Ontario Health atHome guidelines.
Who is Eligible for the FMHC Program?
To qualify, patients must first meet the criteria for traditional home care services and have a care plan developed by Ontario Health atHome. Eligible patients fall into one of these categories:
- Children with complex medical needs
- Adults with acquired brain injuries
- Patients receiving home-based education
- Patients in extraordinary circumstances
