Hospice & Palliative Care
Our Hospice and Palliative Care offers support to you and your loved one during this sensitive stage of life. Our care focuses on comfort, dignity, and peace, allowing you to spend meaningful time together in familiar surroundings. This service expands on our Home Care and Personal Support service placing a strong emphasis on intensive personal care rather than general household tasks.
- Delivered by experienced, certified caregivers with specialized palliative training.
- Care is coordinated closely with the client’s Ontario Health at Home palliative team.
- Registered Nurses can be added for scheduled clinical tasks, such as advanced pain management or wound care.
Included Services
Client-Focused Comfort & Personal Care
- Daily health monitoring and reporting to the palliative team
- Assistance with pain management per care plan
- Regular sponge bathing, oral care, and personal hygiene
- Positioning and repositioning to maintain skin integrity
- Support with dietary needs
Coordination & Support
- Communication with doctors, healthcare professionals, and family members
- Maintaining dignity, privacy, and respect
- Arranging home medical equipment and supplies
- Bridging gaps in public services to ensure consistent, reliable care
